Telehealth Concent

“Telehealth” (also referred to as virtual consultation or remote care) involves the delivery of health and wellness services through secure electronic communication between a licensed medical provider or clinician affiliated with NPOC Wellness and Hydration Clinic (“Provider”) and a client (“Patient”) who are not in the same physical location. Telehealth may be used for consultation, assessment, follow-up, education, or other components of care and may include, but is not limited to:

  • Review and discussion of data transmitted from monitoring or medical devices.

  • Electronic transmission of clinical records, medical images, or personal health information between the patient and the Provider;

  • Telephone discussions related to care coordination or follow-up;

  • Live video consultations conducted via a secure video conferencing platform;
     

The telehealth platform used by NPOC Wellness and Hydration Clinic incorporates industry-standard encryption and security protocols to protect the privacy and security of all transmitted health information.

Patients participating in telehealth understand that while every reasonable effort is made to maintain confidentiality and data protection, telehealth services carry potential risks including technical failures, interruptions, or unauthorized access despite security safeguards.

By engaging in telehealth services, patients consent to receive consultation or care through these electronic means and acknowledge understanding of these benefits and limitations.
 

STATEMENT OF POTENTIAL RISKS AND BENEFITS 

Potential Benefits of Telehealth Services:

  • Can be easier and more efficient for you to access clinical care and treatment from a Provider. 

  • You can obtain clinical care and treatment at times that are convenient for you. 

  • You can interact with a Provider without the necessity of an in-office appointment.  
     

Potential Risks of Telehealth Services:

  • Information transmitted to your Provider may not be sufficient to allow for appropriate clinical decision making by the Provider. 

  • The inability of your Provider to conduct certain tests or assessments in-person may in some cases prevent the Provider from providing a diagnosis or treatment or from identifying the need for emergency clinical care or treatment for you. 

  • Your Provider may not be able to provide some services or treatment for your particular condition via telehealth. In such cases, your Provider may recommend an in-person visit or refer you to another qualified professional or facility.

  • Delays in clinical evaluation/treatment could occur due to failures of the technology. 

  • While NPOC Wellness and Hydration Clinic uses platforms with industry-standard security measures, there is always a small risk that technical safeguards could fail, potentially resulting in a breach of privacy. In such an event, you would be notified promptly in accordance with state and federal regulations.

  • Given regulatory requirements in certain jurisdictions, your Provider’s treatment options may be limited.
